docs(01-02): complete provider abstraction plan

Add summary documenting provider abstraction implementation:
- Provider interfaces in src/lib/providers/types.ts
- Provider registry in src/lib/providers/index.ts
- Phase 1 complete, ready for Phase 2: Model Discovery
